For renewal of the registration of a trademark or renewal with a surcharge of registration of a trademark or for the restoration of a trademark, you need to file a common form TM-R.
Notice as to renewal of the trademark shall be sent by the trademark office at any date not less than one month and not more than three months before the expiration of the last renewal of the trademark.
